What is Continuing Care?
Long Term Care: Supports individuals with more complex health needs whose care cannot be safely provided in their own home, or in designated supportive living. Long term care facilities are sometimes called nursing homes.
What is it? Designed specifically for individuals with complex, unpredictable medical needs who require 24 hour onsite Registered Nurse assessment and/or treatment
Who is it for? Individuals who have highly complex and unpredictable health needs whose care cannot be safely provided in their own home or in designated supportive living
Who provides care? Registered Nurses, Licensed Practical Nurses, Health Care Aides, and other healthcare professionals depending on your needs
The Care Team is comprised of all the individuals who are providing care, as well as you and / or your family. The individuals providing care may include:
• Dietitian • Health Care Aides • Licensed Practical Nurses (LPN) • Mental Health Workers
• Occupational Therapists • Palliative Care Consult Team • Pastoral Care • Pharmacist • Physician
• Physical Therapist • Recreation Therapist • Registered Nurses (RN) • Social Worker • Therapy Aides / Assistants / Volunteers
The Care Team is responsible for planning, coordinating, evaluating and delivering quality, resident focused care / treatment. They are supported by other dedicated staff that provide management, maintenance, housekeeping, laundry, food / nutrition and other support services. The Care Team meets within 6 weeks of a resident’s admission to ensure the plan of care is achieving positive outcomes. If positive outcomes are not being achieved, then the plan will be revised. The Care Team then meets to review and update the Care Plan at least every three months or sooner if a resident has a change in their health status, such as an acute illness, a transfer back from hospital or at the request of the resident / family / substitute decision maker. Please note: Care Team member participation, in regular reviews and updating of care plans, may be formal or informal and may vary depending on their involvement with the resident and availability
